Case Manager

n  Offer
mutual planning, problem solving, and full case management assistance to
clients

n  Provides
SSP and career assessment and SSP Plan development

n  Facilitates
SnagPad registration

n  Refers
clients to vocation training and job skills training

n  Offers
referrals to community resources and services

n  Monitors
participation and attendance

n  Provides
support services and addresses client need

n  Follow
up with clients to ensure that their needs are met and questions and concerns
are resolved

n  Interact
with other team members to provide expertise and assistance in resolving
participant issues

n  Maintain
accurate and timely case notes on all client contacts and document activities





















Inform project
staff of any barriers that they identify and that are preventing the client
from achieving program goals
