Case ManagerAPPLY FOR JOB

• Offer mutual planning, problem-solving, and full case management assistance to clients
• Provides SSP and career assessment and SSP Plan development
• Facilitates Snag Pad registration
• Refers clients to vocation training and job skills training
• Offers referrals to community resources and services
• Monitors participation and attendance
• Provides support services and addresses client need
• Follow up with clients to ensure that their needs are met and questions and concerns are resolved
• Interact with other team members to provide expertise and assistance in resolving participant issues
• Maintain accurate and timely case notes on all client contacts and document activities
• Inform project staff of any barriers that they identify and that are preventing the client from achieving program goals
