ï® Provides SSP and career assessment and SSP Plan development
ï® Facilitates SnagPad registration
ï® Refers clients to vocation training and job skills training
ï® Offers referrals to community resources and services
ï® Monitors participation and attendance
ï® Provides support services and addresses client need
ï® Follow up with clients to ensure that their needs are met and questions and concerns are resolved
ï® Interact with other team members to provide expertise and assistance in resolving participant issues
ï® Maintain accurate and timely case notes on all client contacts and document activities
ï® Inform project staff of any barriers that they identify and that are preventing the client from achieving program goals
